Forms, Forms, Forms!!!

We are all getting geared up for the upcoming year and need some information from you. Thank you for taking the time to fill out this paper work and turning it into the office. Don't forget to check the status of your Safe Environment Training.
  1. Read the Parent and Student Handbook - There have been  a few modifications from previous years.
  2. Print the Policy Handbook Aknowledgment Form and Sign.
  3. Print the Parent Computer Consent Form and the Student Computer Consent Form.  These are signed by both parent and student.
  4. Print the Emergency Care Form.  Fill out all fields and sign accordingly.
  5. If you are a new family, please print, fill out, and sign the Home Language  Survey. You must fill out one survey per student. (This is a one time survey that transfers with your student through their education.  It can also transfer from school-to-school.)
Please have these filled out, signed, and returned to school by the first day of school.
